Thanks Michael. My iscsi config on the host is empty and I tried uninstalling ALL the recent updates & restarted the server but to no avail. I've posted a support request as you suggested.

It worked well on a 2012R2 server for about a year, and then the server got updated with some recent Win Updates and after a reboot I had exclamation marks on the devices.
