The iSCSI Initiator (Server 2012 R2) event log loop with 2 event.

event 20 , iScsiPrt
Connection to the target was lost. The initiator will attempt to retry the connection.

event 34 , iScsiPrt
A connection to the target was lost, but Initiator successfully reconnected to the target. Dump data contains the target name.

I tried to remove all disk and create new disk still looping the errer.

Finally I found it is my server problem,
I tested on the other server it run smoothly

The story is i want to remove the cluster feature in one of my server.
it not success ,just hold the loading screen.

Then I force reboot the server , after that the cluster service was uninstalled
I think the server is okay but cannot connect to the target.

Finally , i use the secondary server to connect the target without any problem(tested many times)
